The City of Muncie is concerned about issues affecting our community. This reporting forum has been designed to address your concerns and to help make Muncie a better community for all. If you have a problem or concern you would like to report, please select the type of problem from the list below. If you are are trying to report a problem that is not listed below, please email the Office of the Mayor to better determine where your request should be directed.

All reports will be forwarded to the appropriate department, and an investigation or necessary action will be initiated. It is important to include as much detail as possible to better assist with identifying the resolution to the problem. Identify the location of the problem you are reporting on the appropriate reporting form. Please include the street name; the street direction; and approximate street address (or nearest cross-street).

Including your contact information is essential for necessary follow up by the appropriate responding department, and so that you may be available in case we need to clarify the information you've entered.
